Earn at least 3 points from each category.  15 total points needed. 
All due A Track - Friday, May 13, 2016 and B/C/D Tracks – Friday, June 3, 2016

Participate in a before school music program (band, orchestra, choir).
Receive all ‘A’s in the Letter Grade area of the Report Card.

Receive all ‘E’s on the Work and Social Skills area of the Report Card.

Pass a multiplication timed drill (100 facts in less than 3 minutes with 95% accuracy).
Actively participate in Girl or Boy Scouts or another similar group.
Attend Fall AND Spring Parent Teacher Conferences with your parents.

Pass a division timed drill (100 facts in less than 3 minutes with 95% accuracy).
Serve as a lunch worker.
Participate in an organized physical activity (dance, gymnastics, soccer, basketball, football, etc.).  See your teacher in advance for a record sheet.  You must document 10 hours and have it signed by your coach or instructor.
(2)  Complete one book report from six of the following eight genres:  biography, realistic fiction, adventure, mystery, historical or historical fiction, fantasy or folk tale, animals or sports, science fiction, or reference.  See Mrs. Berven/Thomas for book report form.  All due by May 6.
Perform 10 service hours for Elk Meadows Elementary, a sixth grade teacher, or another teacher in the school.  SERVICE ACTIVITIES MUST BE PRE-APPROVED BY YOUR TEACHER!  You must document your hours and have it signed by the teacher or school person you helped.
Participate in an organized cultural activity (art, voice, theater, musical instrument, etc.).  See your teacher in advance for a record sheet. You must document 10 hours and have it signed by your coach or instructor.
(2)  Prepare a science project  TOPIC MUST BE PRE-APPROVED BY Mrs. Boehme/Goff  Curriculum topics include Phases of the Moon, Earth’s Axis and Seasons, Solar System, Universe, Microorganisms, Heat, Light, or Sound.  Requirements will be presented and demonstrated in class. 
Actively participate in Student Council.  Attendance at all required meetings or assignments. Verified by the adult in charge.
Submit at least 5 essays on the Leadership Habit of the month to Mrs. Packard.  Each essay needs to be handwritten and at least one page long.  If you skip lines, the essay needs to be at least two pages long.
Write four additional essays in the following writing forms:  personal narrative, description, how-to, and persuasive.  You must have an edited rough draft and published copy to turn in.
Published copy must be at least one page typed, double-spaced, in 12 point font.  All essays due to Mrs. Packard.
Listen to or read to a younger sibling or other child for 20 minutes a day for a week. 
Have fewer than 6 absences during the year.  This DOES include vacations and educational leave.
Improve your reading level from the beginning to the ending of the school year.
Make dinner for your family once a week for a month. 
Have fewer than 6 tardies.  This DOES NOT include school announced tardies because of  inclement weather or bussing conflicts.
(2)  Complete the Math Vocabulary packet due to homebase teacher.
Rake leaves or shovel snow for someone in need in your neighborhood (not your house).
Set and fulfill your own challenging and measurable goal for sixth grade.  GOAL MUST BE PRE- APPROVED BY YOUR TEACHER.  Goal must be set and approved by the teacher before Christmas.
Represent your class in the Spelling Bee or Geography Bee.
Write a letter to someone you know who does not live in Utah.
Pass off 4 of the 5 Presidential Fitness Award requirements.
Earn 80% or higher on a Math Benchmark Test.
Participate in a service project with your family.
Earn the National Fitness Award.
No 1’s on your report card.
Write a Thank You note to someone who has done something nice for you in the last month.
Participate in the Sixth Grade Music Program.
Increase at least one letter grade in one term.
Clean your room every day for a month without being asked by a parent.
